​Plan Terms
This program is not refundable if the patient enrolls in a dental insurance program during the one year term of this plan. 
This program is not retroactive. The plan and its benefits begin once the plan is purchased. 
In order to receive the discounted fees, payment in full is required at the time of service. Regular fees will be charged for any treatment not paid at the time of service. 
This plan cannot be used with any other discounts or insurance coverage. Discounts are taken off regular office fees.
